Community Health Specialist 1 or 2

Delaware Public Health District

The Delaware Public Health District seeks a F/T Community Health Specialist (CHS 1 or CHS 2) to perform duties related to assessment/planning/improvement, community engagement and health education within the Community Health Division.  This position will begin as a member of the COVID-19 response team and be responsible for interviewing, evaluating and monitoring clients, and providing education, guidance and isolation/quarantine requirements based on Centers for Disease Control and Prevention (CDC), Ohio Department of Health (ODH), and DPHD.  This position will then permanently move to the Community Health Division and be responsible for coordinating various strategies within the Community Health Improvement Plan related to Addiction, Mental Health, Chronic Disease, and Social Determinants of Health.

Job duties will include but not be limited to: assessing need, action planning, program implementation and evaluation, grants management, health education outreach, conducting speaking engagements, policy development and coordinating community coalitions.

Requirements:

CHS 1: Bachelor's degree in public health, education, health promotion, social work, nutrition and dietetics, nursing or other related field preferred.  Less than one year of relevant experience required.   

CHS 2: Bachelor's degree in public health, education, health promotion, social work, nutrition and dietetics, nursing or other related field preferred.  One to three years’ of relevant experience required.   

Pay and Benefits:

  • CHS 1 starting pay is $19.89/hr, may be adjusted commensurate with experience
