Position: Community Health Worker- posted 9/29/20 starting at $13.83
This position is responsible for conducting outreach in the community to find participants for the Healthy Start program. Responsibilities include enrolling women and their families into the Healthy Start program at various reproductive stages, including pre-conception, inter-conception, and prenatal. Healthy Start’s goal is to help improve women, infant and children health and to reduce poor birth outcomes by providing individual services and community support to reduce disparities in health and access to health care services.
Basic Qualifications:
- High school diploma or G.E.D.
- Proficient oral and written communication skills
- An understanding of the issues that women from minority, disadvantaged, or underserved populations face when accessing health care
A valid OH/MI driver's license
Preferred Qualifications:
Certification as a Community Health Worker
Responsibilities:
- Conducts neighborhood outreach activities, including partnering with community members and neighborhood leaders, to target vulnerable, underserved populations eligible for the Healthy Start program.
- Enrolls eligible women and their families in the Healthy Start program, and connects them to medical care, including prenatal care.
- Works with clients and their families throughout the pregnancy to reduce barriers to care.
- Utilizes the Partners for a Healthy Baby curriculum as the standard of care for home visitation and other interactions.
- Educates clients about maternal health and the importance of prenatal care; nutrition and vitamins; safety needs; risks of smoking, drugs and alcohol; signs of labor; stress and depression; and issues related to fatherhood.
- In the last trimester of the pregnancy, helps clients prepare for delivery; discusses signs of labor, acquire a car seat, assesses the safety of the home and ensures that the baby has an appropriate crib.
- Works with insurance and Medicaid Managed Care to ensure appropriate coverage for clients.
- Connects the client to social and health services as required; assists in the completion of paperwork.
- Consistently completes appropriate documentation in a timely fashion, follow the HIPPA law and maintain clients confidentiality
- Communicates with client's health care providers and insurers as necessary to provide coordination of care.
