Job Description: We seek an enthusiastic individual to join our team as a full-time Registered Nurse, working 40 hours per week. Responsibilities:
Under the direction of the Clinical Supervisor and Clinical Director, the responsibilities include, but are not limited to:
- Participates in direct and indirect patient care by utilizing the nursing process, designated scope of practice through data collection, applying critical thinking skills and motivational interviewing techniques to promote the delivery of safe, quality, and age-specific care
- Collaborates for Medicare Annual Wellness Visit by familiarizing self with Medicare Annual Wellness Visit CMS requirements Identifies patients that may benefit from Medicare Annual Wellness visits utilizing data analytics software and EHR (Electronic Health Records)
- Schedules patients to close gaps in care and follow up with patients who are no-shows and/or cancel an appointment
- Conducts preventative screenings and assessments as needed
- Collaborates for Transition of Care Management by familiarizing self with Transition of Care Management requirements and being able to perform within the given scope of practice
- Identifies patients that necessitate a Transition of Care Management visit utilizing data analytics software and EHR
- Schedules patients to close gaps in care and follow up with patients who are no-shows and/or cancel an appointment
- Conducts preventative screenings and assessments as needed
- Completes post-inpatient and emergency room discharge calls within 48 hours of notification
- Completes a care workflow utilizing CMS Transition of Care Management requirements
- Schedules a follow-up visit with a provider within 7 business days or according to CMS Transition of Care Management requirements
- Schedules patients for tele-visits or face-to-face visits for healthcare needs
- Collaborates with patients and healthcare teams to establish individualized, objective, and time-specific action steps to assist the patient in identifying areas to improve their overall health.
- Assesses/reassesses patient’s condition based upon data collection and interview of patient and/or family, diagnosis, diagnostic results, and relevant data from other healthcare providers.
- Provides ongoing support and evaluations of progress toward the patient’s health goals and education about their specific conditions/needs
- Acts as an advocate for patients and helps them obtain their maximum level of health.
- Involves patient’s family, community, and other natural supports when authorized by the patient in the attempt to promote a maximum level of independence
- Maintains EHR documentation and data concerning patient interactions.
- Works with partnered insurers to participate in quality incentive programs with the goal of revenue maximization and value
- Works with the Quality and Clinical Director to review policies, identify the need for process improvements, and collaborate to design new processes.
- Review patient and other medical data and analyze processes used to provide care. Using that information, they work to identify areas for improvement.
- Collaborates with the Population Health Navigator to create timely and accurate reports of quality goals, able to communicate the necessity and importance of quality goals with all staff
- Closes care gaps around specialty and chronic disease
- Assists with the development and staff training of EHR workflows
- Coordinates and resolves staff requests related to EHR maintenance for projects and issues
Qualifications:
- Certificate completion of an accredited nursing program, required
- Licensed by the Ohio Board of Nursing as a Registered Nurse, required
- Minimum of one year of experience as a Registered Nurse, preferred
