NOW HIRING: Community Health Planner
The Community Health Planner is responsible for the development, facilitation and expansion of Community Health Action Teams (CHATs) at the local level including assessing community needs and creation of new programs; identification of possible barriers to programs; budget development of programs; development and recommendation of policies and procedures; monitoring the implementation of projects; evaluating the effectiveness of projects; and community engagement.
Duties Include:
- Administers assigned public health programs in local communities; researches potential health risks; conducts on-site assessments; investigates reported cases and complaints; provides data and status reports;
- Assess the health needs of designated populations and communities, analyze and evaluate programs and policies, and use the assessment information to develop plans to carry out FCPH public health objectives;
- Communicates with other agencies, schools and/or organizations to identify community needs and promote programs;
- Works with local community members and leaders to identify local funding to support interventions;
- Facilitates community teams and events, some of which fall outside of normal business hours;
- Other related duties as assigned
Requirements:
- Bachelor’s degree in public health, business administration, health sciences, or related field – Master’s degree preferred
- Two to three (2-3) years of public health experience or equivalent
- Minimum 1 year facilitation experience
- Ability and willingness to work outside of normal business hours
- Must possess a valid driver’s license, and be insurable through FCPH’s provider; some evening and weekend hours required.
- Non-smoker.
Hiring Salary: 19.24/hour - $24.06/hour. This is a non-exempt position.
