Communicable Disease Nurse (PHN 2)

Job Duties:

    • Planning, organizing, implementing and evaluating communicable disease investigations;
    • Using electronic disease reporting systems following CCPH, Ohio Department of Health, and Centers for Disease Control and Prevention guidelines and protocols;
    • Educating clients on health issues related to disease prevention;
    • Maintaining data for preparing reports, program planning, evaluating services and quality assurance;
    • Administering vaccinations to children and adults;
    • Participating in emergency preparedness activities.     

 

Minimum Qualifications:

  • Bachelor’s degree from an accredited school of nursing (preferred) and/or two years’ experience, or equivalent combination of education, training and/or experience;
  • Ohio license to practice as a Registered Nurse (Section 4723.01 of Ohio Revised Code);
  • Knowledge and skill in vaccine administration (preferred);
  • Must be able to work independently and as part of a team;
  • Possess strong organizational skills, have basic computer skills, and be able to manage multiple tasks effectively;
  • Must have and maintain a valid driver’s license;
  • Required to have regular and predictable attendance.
