Community Health Worker

FUNCTION

Lorain County Public Health (LCPH) is seeking to hire a full-time Community Health Worker whose primary duties will be within the Community Health Davison. Under the general direction of a Nursing Supervisor, will work in public health programs providing support, education and resources for clients, residents and community agencies in the areas of substance use, prescription assistance and maternal child health.

 ESSENTIAL JOB DUTIES

  1. Function as a member of an interdisciplinary team that performs activities related to substance use and overdose prevention, prescription assistance, infant safe sleep and maternal child health.
  2. Assist with Naloxone (Narcan) activities which include but are not limited to: inventory management, kit assembly, data entry, training, community outreach, distribution, completing mail order request, and monitoring of Naloxone service entities.
  3. Assist with data entry for Overdose and Suicide Fatality Review Committee.
  4. Educate families on infant safe sleep practices, and portable crib assembly to reduce the risk of sleep-related injury and Sudden Infant Death Syndrome (SIDS).
  5. Communicate in person, or by phone, with Lorain County residents in need of prescription drug assistance.
  6. Assist residents with completing a submitting required enrollment forms for the Drug Repository Program.
  7. Follow-up with physicians and pharmacies to ensure client prescription needs are met; link clients to manufacturer medication assistance programs.
  8. Assist with coordinating agency presence and/or attending community-based events to promote relevant agency services.
  9. Perform administrative functions including, scheduling of meetings, compiling and distributing meeting minutes, and maintaining an organized system using Google Suite (i.e. drive, calendar, docs, sheets, Gmail, etc)
  10. Participate in team meetings to review and evaluate program needs an actitivies.
  11. Attend webinars, and in-person meetings.
  12. Other duties as assigned

MAJOR WORK CHARACTERISTICS

Knowledge of:  health equity issues affecting multiple intersections of client identity (i.e. race, age, class, LGBTQ+); local community resources and services; working with the target population providing culturally appropriate services; trauma-informed care; basic computer skills including email, Internet, Microsoft Office Suite, and Google Suite.

Ability to:  work independently and as a member of an interdisciplinary team conducting outreach to community members and partner agencies; initiate conversations with the public and individuals; demonstrate good listening and interpersonal skills that support patience, resourcefulness, enthusiasm, flexibility, and empathy; record, organize, and present data with accuracy, thoroughness, and attention to detail; communicate effectively in written or oral form; hold a flexible schedule.

Minimum Qualifications: Must possess a current Certification in Community Health from an accredited program, or become certified within six months of hire. Must have a valid Ohio driver's license and be able to travel throughout the workday with reliable transportation. 

Physical Requirements and Unusual Working Conditions: Be able to lift objects of weight up to twenty-five (25) pounds. This position is office-based at LCPH and; requires travel across Lorain County and occasionally across Ohio. Must be able to sit and stand for long periods and bend and stretch to access various physical filing systems and execute community outreach activities.

Salary: $20.19 - $25.76
