Case Manager
Public Health – Dayton & Montgomery County
Department: Addiction Services/ Medication Assisted Treatment (MAT)

Hours: Full-time, M - F 8 am - 4:30 pm
Starting Salary: C42 $20.42/hour- $28.10/ hour $42,473.60/Annual - $58,448.00/Annual
Minimum Qualifications:
- A Bachelor’s degree in Social Work, Substance Abuse or related field
- Current valid Ohio driver’s license & access to a motor vehicle
- Licensed Social Worker or Professional Counselor required
Preferred Qualifications:
- Two years of experience in chemical dependency preferred
Responsibilities include but not limited to:
- Performs a bio/psycho/social assessment of client’s mental and physical health, evaluating their substance abuse treatment needs, situation, concerns, and barriers to accessing services
- Monitors substance abuse cases and provides personal support and crisis intervention when needed; Recommends counseling services for clients in individual or group settings; Facilitates groups
- Facilitates client’s coordination of care, develops a plan of action, sets goals, and collaborates with external services to facilitate case coordination and share information about available professional and community resources; Educates clients about needed community resources
- Maintains confidentiality, respect client’s privacy, routine, and independence as much as possible
- Contacts clients to ensure they are continuing to meet goals and make positive lifestyle choices; Evaluates client satisfaction and quality of care provided; Makes recommendations to improve quality of care as needed
- Consults with a multi-disciplinary team; Routinely completes client records, documentation, and statistical reports within required timelines and according to program and accreditation standards and protocol
- May be required to play an active role in the event of a public health emergency, which may include changes in responsibilities and working hours
