Position Title: PHN Supervisor: Communicable Disease
Entry Salary: $28.50
*3.5% pay increase upon successful completion of probation.

Full-time position. Must hold a current, valid license to practice as a Registered Nurse in the State of Ohio, Valid Ohio driver's license with state minimum insurance and an acceptable driving record. Current CPR certification. Diploma, Associate or Bachelor of Science in Nursing from an accredited school of nursing required. Experience in public health nursing, related nursing experience or BSN preferred. Excellent benefits package. A 180-day probationary period is required.
Starting salary is $28.50 with a 3.5% pay increase upon successful completion of probation making the new rate $29.50. This position may be remote for 40% of the time with performance measures met or exceeded.
