The Holmes County General Health District is seeking an experienced a Community Health Worker to perform a variety of duties within a community setting. Job duties for this position are not limited to but including assisting with outreach immunization clinics, health screenings, coordinating the GVO grant, communicable disease prevention and investigations.
The Community Health Worker position requires a current CHW certification. CHW, LPN or ADN preferred. Valid Ohio Driver’s license and proof of insurance required. Experience in Public Health preferred. The successful candidate would be self-motivated, achieve CHW certification within 6 months of hire, possess strong writing and communication skills, bilingual skills (Spanish or Pennsylvania Dutch) is a plus, a professional attitude, willingness to expand their knowledge as a healthcare professional, and a passion for community health.
