Salary Range: Pay scale 3
Hours of Work: 8:30 am – 4:30 pm. Will require some evenings and weekends

Job Duties:

Provides psychosocial assessments of children and/or families to determine social, economic and/or environmental needs following local, state, and federal grant guidelines.

Available to clinical areas as needed.
Provides support and limited counseling to families and assists in meeting concrete needs such as housing, food, public assistance, etc.
Works collaboratively and cooperatively as part of a multi-disciplinary team.
Responsible for referral and follow-up with individuals and families.
Maintains clinical records and documentation accurately.
Provides complete and accurate referral/connection requests for clients.
Will travel to and from clinic locations as needed.
Assist with clerical and supportive activities as required.
Assist the nursing administration in the planning and development of programs to meet the needs of the community.
Other duties as assigned by the Nursing Director/Unit Manager/ and or the Health Commissioner.

Qualifications:

Bachelor’s degree in social work.

Licensed by the Ohio Board of Counselors and Social Workers.
Minimum 2 years recent experience in working with mothers, children, and families.
Excellent verbal and written communication skills.
Working knowledge of Stark County Services and systems.
Skills necessary in teaching, public speaking, public relations, writing and communication.
Must have: reliable transportation / valid Ohio Drives License / proof of auto insurance/ fully vaccinated for COVID-19.
