Case Management Nurse

The Children with Medical Handicaps (CMH) Program utilizes Registered Nurses as Public Health Nurses (PHN) employed by local public health agencies to serve as local care coordinators within the community. CMH works closely with local health departments (LHD) that provide services to children with special health care needs and their families. CMH views the PHN as an essential member of the health care team and is a vital link in coordination of health care services and service delivery for children with special health care needs (CSHCN). PHN services are provided by the PHN in both the community and home setting. In the community setting, the PHN provides case-finding, community assessment input and nursing leadership on local collaborative committees. The PHN also contributes to policy development and quality assurance within the agency. Additionally, PHNs monitor and provide family centered care and services to individuals and families that include an initial home visit with annual follow-up home visits, education of the client and family, clinical services, and coordination of services. The PHN works with designated hospital-based providers, Team Service Coordinators assigned to specialty teams located at the various pediatric hospitals.
