Community Health Worker Job Description

Reports to: Director of Mission
Summary: Susan G. Komen Columbus seeks a community health worker (CHW) who will focus on bringing the mobile mammography unit to operating at full capacity and be integrated into all aspects of the region’s breast healthcare initiative. The CHW will collaborate with the OU program, its fiscal agent program, and OhioHealth’s standing and mobile mammography facilities. The CHW will build community relationships with agencies, churches, work sites, primary care facilities and FQHCs to identify and recruit clients appropriate for screenings. The CHW will provide education, obtain client information to enroll them for support, follow-up with risk assessment and schedule clients. The CHW will obtain release forms so the results can be shared with the CHW and follow-up support can occur, if necessary. They will manage clients and partnerships referring to screening, provide follow-up care coordination services for patients in need of referral for diagnostic services and manage referrals to other community-based agencies, as needed. The CHW will track all client recruitment, outreach, education, follow-up and referrals, and perform other community outreach duties in the southeast Ohio region, as assigned. This position will work remotely with occasional travel to Komen Columbus offices in Westerville, Ohio and weekly travel throughout the Southeast Ohio region.
Objectives:
- Increase utilization of regional BCCP (baseline: 4697 or 6% of eligible women in SE Ohio Region)
- Increase utilization of O’Bleness mobile unit to more dates and full schedules (500 additional patients screened (equivalent of 28 additional dates at full schedules)
- Increase screening among Medicaid Managed Care Plans’ non-compliant patients (pending funding and business agreements from MCPs to do community outreach to their plan participants)
- Communicate with at least 10 clients per day
Essential Duties and Responsibilities:
- Build sites and partner community-based relationships with businesses, agencies, primary or other specialty care facilities and more
- Coordinate to schedule mobile screening events with OhioHealth mobile mammography unit
- Promote mobile screening events and recruit, educate and schedule participants
- Coordinate with other regional Komen and cancer resources as needed
- Log client information in management software
- Obtain releases for follow-up with clients
- Provide risk-assessment information and referrals for genetic counseling
- Recruit and enroll eligible clients in Breast and Cervical Cancer Project
- Establish supportive relationship with nurse navigators in the region
- Obtain patient satisfaction surveys for evaluation, success stories, advocacy and future programming
- Communicate regularly with Komen Columbus staff and partnering programs
- Assist as needed in support of the Southeast Ohio Race for the Cure
Requirements:
- Vocational, technical training or associate degree in community health work or social work
- Ability to take initiative and self-start
- Detail-oriented, must be extremely organized
- Excellent verbal and oral communication skills
- Proficiency in Microsoft Word, PowerPoint, Excel and email
- Ability to learn client management software system
- Familiarity with breast cancer a plus
- Must be able to drive own vehicle frequently in the 15-county service area
