Job Duties:
Care coordination of the Children with Medical Handicaps (CMH) program; conducting home/ telehealth visits with families; assessing family needs and providing support, information, and referrals as necessary; completing all nursing documentation in the electronic medical records and billing as required by CMH; performing chart reviews and implementing quality improvement measures. Active role in providing infant, children, adult, and international travel vaccinations in CCPH immunization clinics on a routine basis; participating in the immunization program planning and implementation. Conducts management on all reported cases of Sudden Infant Death Syndrome (SIDS) and Childhood Lead Poisoning occurring in Clermont County. Participates in outreach activities to educate clients, providers, agencies, and the community about CCPH Community Health Service programs. Actively participates in continuous quality improvement with the agency through individual work plans and/or projects. Participates in activities to ensure and maintain accreditation status. The individual filling this position will also be trained in basic CCPH-related emergency response procedures and considered essential for any public health emergency response and disaster recovery.
Minimum Qualifications:
- Ohio license to practice as a Registered Nurse (Section 4723.01 of Ohio Revised Code) (Required)
- Bachelor’s degree from an accredited school of nursing (Required)
- Six months experience in public health nursing or equivalent nursing experience (Preferred)
- Cardiopulmonary resuscitation (CPR) certification (American Heart Association or equivalent) (Preferred)
- Knowledge and skill in administering vaccinations to infants, children, and adults (Preferred)
- Care Coordination and home visiting experience (Preferred)
- Knowledge and proficiency using an Electronic Health Record system (Preferred)
- Independent worker
- Skills needed in organization, networking, and basic computer skills
- Must have and maintain a valid driver’s license
- Required to have regular and predictable attendance

Clermont County Public Health offers a generous fringe and benefits package to all employees:
- Sign-On Bonus- New Employees receive a one-time $500 sign-on bonus after completing their 180-day probationary period. After one year of service, staff will receive an additional $1,000 bonus.
- Attractive Schedule- We follow an attractive work schedule of 8:30 am to 4:30 pm, Monday through Friday. Full-time employees have the option of a four-day (10 hr./day) workweek once the probationary period is complete. Actual workdays will be negotiated during hiring for part-time employees.
- Remote Work Option- This position can work remotely 1-2 days a week after the probationary period.
- Paid Leave (312-888 hours per year; full-time employee) (119-616 hours per year; part-time employee)
- Vacation- Vacation starts at two weeks annually with up to 5 weeks per year at 20 years of service.
- Sick- Sick leave accrues at 4.6 hours per 80-hour pay period, which amounts to 15 days per year for full-time employees. Sick leave accrues at 0.0575 hours per hour worked, which amounts to 9 days per year for part-time employees.
- Holiday- CCPH observes thirteen paid holidays. Part-time employees receive holiday pay for those that fall on their regularly scheduled workday.
- Personal Leave- Full-time employees earn one personal day per year and can convert sick leave for up to five additional personal days per year.
- Parental Leave- After one year of employment, employees are eligible for six weeks of paid leave following the birth or adoption of a child.
- Military Leave- National Guard, Defense Corps, Naval Militia, and all U.S Armed Forces reserve members are authorized up to 176 hours of paid leave per calendar year for training purposes.
- Public Employees Retirement System (PERS) – All employees contribute 10% of their salary to PERS, and the agency contributes 14%. The agency estimated value based on the above salary range from $7,767 to $9, 321 (full-time) annually and $4,660 to $5,593 (part-time) annually. Employees do not pay federal social security tax from income; all contributions go directly towards PERS.
- Deferred Compensation Plans– Employees may contribute to tax-deferred supplemental retirement accounts.
- Tuition Assistance Program – CCPH offers tuition assistance to all employees after one year of service, up to 90% reimbursement.
- Student Loan Forgiveness- Public service employees may be eligible for student loan forgiveness through the Public Service Loan Forgiveness (PSLF) Program.
- Sick Leave Rewards- Employees with low sick leave usage traditionally receive an annual sick leave bonus of up to $800 for full-time employees and $480 for part-time employees. They are also eligible for quarterly drawings, which could earn up to an additional $240 per year.
- Fleet Vehicle – CCPH provides fleet vehicles to all staff and covers all fuel and maintenance. Fleet vehicles are not take-home vehicles.
- Health Insurance (Full-time only) – Employees may choose coverage from single to full family benefits based on eligibility. Depending on the selection, the agency pays 81% - 87% of coverage. The value of health insurance ranges from $6,569 to $25,232 annually. In addition, employees selecting high deductible plans can receive $600 – $1,200 per year for a health savings account.
- Dental Insurance (Full-time only) – Optional for employees to pick up, for $12.25 – $37.51 per pay period.
- Vision Insurance (Full-time only) – Optional for employees to pick up, at $2.82 – $8.08 per pay period.
- Life Insurance – Agency provides $25,000 annually, with no charge to employees.
- Supplemental Life Insurance – Employees may purchase up to $250,000 for employees, $100,000 for spouses, and $20,000 for children.
- Accident and Critical Illness Insurance – Coverage is available to be picked up by the employee.
- Long-term Disability – Agency picks up the first five years until the employee becomes vested in PERS.
