POSITION OVERVIEW
The Social Worker/Behavioral Health Coordinator provides resource navigation and limited therapy services as a member of a multidisciplinary primary care team. Under the supervision of an LISW, provides person and family-centered, culturally-competent evidenced based treatments through face-to-face therapy with individuals and or families including, but not limited to: diagnostic assessment, treatment of mental, emotional, and substance use disorders. Special Note: This is a grant funded position.
ESSENTIAL DUTIES
Social Worker Services- Resource Navigation
• Provides assessment, consultation, and brief interventions for behavioral health and substance use disorders;
• Successfully completes all required federal and/or state training for certified application counselor or equivalent;
• Assists individuals with health insurance eligibility determination and enrollment, including Medicaid and Marketplace health plans;
• Collaborates with patients and interdisciplinary team members to identify patient resource needs;
• Assists clients in identifying goals and implementing strategies to address needs and improve overall health and social status;
• Assists with referrals and linkage to agency and community resources;
• Provides advocacy, counseling and support services;
• Maintains accurate, complete, and timely case records, data and supportive materials;
• Collects and analyzes data from clients;
• Collaborates with team members and participates in coordination meetings;
• Develops educational resource information and materials for clients. Behavioral Health Coordinator Duties
• Provides assessment, consultation, brief interventions, and psychotherapy for behavioral health and substance use disorders;
• Creates positive relationships with patients, their families, guardians, or agency representatives;
• Accurately utilizes Electronic Health Records (EHR) for documentation and billing of all data related to patient care;
• Evaluates, coordinates, and facilitates crisis management and stabilization as needed;
• Participates in supervision and Behavioral Health meetings;
• Maintenance of all required licensure, certification, registration, training, behavioral health knowledge (including, but not limited to, new or changing treatments and standards of care), and expertise to meet the needs of patients.
REQUIRED QUALIFICATIONS
• A Bachelor’s Degree in social work, or related field;
• A valid Ohio Social Work license. License must be maintained throughout employment;
• A valid Ohio driver’s license. License must be maintained throughout employment;
• At least one (1) year experience working as a full-time Licensed Social Worker.
PREFERRED QUALIFICATIONS
• Certified Application Counselor (for Marketplace health insurance plans);
• Experience completing applications for Medicaid;
• Experience working with patients who are experiencing substance use disorder;
• Experience providing psychotherapy. KNOWLEDGE, SKILLS AND ABILITIES
• Ability to provide proof of automobile insurance. Must be maintained throughout employment;
• Knowledge of mandated reporting laws; knowledge of human development and behavior; awareness of social, economic, and cultural systems impacting individuals, families, and groups; and awareness of community resources.
• The ability to successfully pass a background check. Final candidates must successfully pass a background check and provide proof of a valid State of Ohio driver’s license as well as current automobile insurance. An individual who poses a direct threat to the health and safety of others in the workplace will be deemed not qualified for this position.
