Performance Measurement and Accreditation Coordinator

The Performance Measurement and Accreditation Coordinator position will manage all accreditation renewal activities for Franklin County Public Health related to the Public Health Accreditation Board (PHAB). This position also plans, organizes, directs, and leads the process of the quality improvement program and program-based performance management. The PMAC is responsible for working with internal and external teams to identify and address health problems, disparities and social determinants of health.

Duties Include:

• Supports efforts in Health Equity, works to identify the root causes of health inequities, health disparities, with a focus on health determinants, as defined by Healthy People 2020
• Manages the department’s process for accreditation including: documentation, staff training; annual accreditation updates and reporting
• Assists in implementation of the CLAS standards as a tool for addressing health inequities
• Develops and implements a program-based performance management system; analyzes requests and defines business needs, develops objectives and work plans
• Conducts research and aligns grant opportunities with the agencies core priorities; writes grant applications and submits to agency or foundation
• Assists with the development of a comprehensive community health assessment (CHA) and a community health improvement plan (CHIP) to assure that the processes align with other community plans that collect and report population health data
• Assists in the development of the agency’s strategic plan with senior staff
• Develops Quality Improvement Plan in collaboration with Director; works with the agency’s performance management committee and leads the quality improvement team.
• Other related duties as assigned

Requirements: Bachelor’s degree in public health, health & human services, health education or related field (Master’s preferred); three (3) years of experience Grant Writing and Quality Improvement, or equivalent. Must possess a valid driver’s license, and be insurable through FCPH’s provider.

Salary Range: $21.18/hour - $26.48/hour (non-exempt position)
