Hours: Full-time, M - F 8:00 am - 4:30 pm (requires some weekend hours)
Starting Salary: I $20.91 - $28.77/hour $43,492.80 II $23.68 - $32.58/hour $49,254.40
Minimum Qualifications:
- A Bachelor’s degree OR an Associate’s degree with two years of nursing experience
- Current valid Licensure from State of Ohio to practice as an Registered Nurse
- Current valid Ohio driver’s license & access to a motor vehicle (mileage is reimbursed at IRS rate)
Preferred Qualifications:
- Bachelor’s degree with four years nursing experience (two of which are in Public Health) OR an Associate’s degree with six years of nursing experience (two of which are in Public Health)
- Experience in a Medicare Certified home care program
- OASIS Assessment experience
- Knowledge of Medicare Conditions of Participation
Responsibilities include but not limited to:
- Conducting nursing assessments on patients to obtain bio/psycho/social data and then develop and implement care plan, and assess effectiveness of care plan
- Provide direct nursing care to patients such as wound care, administer treatments per physician’s orders; provide instruction on disease processes, diet, medications; and record in electronic medical record
- May be asked to float temporarily to other positions as staffing needs arise
- May be required to play an active role in the event of a public health emergency, which may include changes in responsibilities and working hours
