Accreditation Coordinator

Portage County Health District

Full Time/ Part Time:  Full Time – Non-Exempt
Work Schedule: Monday – Friday 8am- 4:30pm. Hours may vary. May require some weekend and evening work.
Salary Range: $22.00- $26.00 Commensurate with experience

Job Description: Collaborate with PCHD management and staff to assure public health practice will meet or exceed required documentation for PHAB Standards & Measures for reaccreditation; create annual reports as required by PHAB

Support and participate in Workforce Development Committee, Quality Improvement Committee, Strategic Plan and other groups or individuals re: organizational assessments, documentation, and/or public health practice issues

Develop and monitor tracking of Performance Management System; work within the Clear Impact Scorecard for agency Performance Management System, agency Strategic Plan, and Community Health Improvement Plan

Contribute to development and monitoring of Community Health Assessment and Community Health Improvement Plan

Collaborate with Division directors and supervisors on special projects and divisional initiatives

Other duties as assigned

Organizational Duties: ICS/NIMS training within 60 days of hire. Facilitate and participate in quality improvement team projects and quality improvement training initiatives as applicable/required and incorporate the principles of quality improvement into daily work activities. Participate and coordinate organizational activities that will support and enhance agency accreditation. Other duties as assigned. Minimum Qualifications:            

Bachelor’s degree related to public/community health.

Five years professional experience in governmental public health.

Excellent written and oral communication skills and ability to multi-task.

Experience in data collection, management, and organizational skills.

Familiarity with PHAB Standards and Measures and Council on Linkages Core Competencies for Public Health Professionals and other professional competency sets.

Computer skills with emphasis on Word, Excel, PowerPoint, Publisher and Adobe Pro

Ability to facilitate large and small groups.

Strong leadership, problem solving, collaboration skills

Ability to work autonomously and as a team player

Pre-Employment Testing Requirements: Criminal Background Check, Pre-employment Drug Testing

Work Environment: General office setting in the Health Department facility requiring sedentary to light work which may include sitting, walking, climbing, reaching, standing, talking, and listening with occasional exposure to adverse environmental conditions. The role routinely uses standard office equipment such as computers, phones, photocopiers, filing cabinets and fax machines. While performing the duties of this job, the employee is regularly required to talk or hear. The employee frequently is required to stand; walk; use hands to finger, handle or feel; and reach with hands and arms. The employee must frequently lift and/or move up to 10 pounds and occasionally lift and/or move up to 30 pounds. Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, depth perception and ability to adjust focus.
