Community Health Worker (HUB Pathways Care Coordinator)

Intermittent, Unclassified, Hourly, Non-Exempt. 37.50 hours per week, Monday – Friday, with a maximum of 1,000 hours per fiscal year. The pay range for this position is $11.63 - $16.11 per hour, based on experience.

Communicate to clients the purposes of the program and help identify and address socio-economic issues that affect their overall health through the development of care management plans and goals. Facilitate client access to community resources, including locating housing, food, clothing, prenatal classes, parenting, and providers to teach life skills, and relevant mental health services. Assist clients in utilizing community services, including scheduling appointments with social services agencies and assisting with completion of applications for programs for which they may be eligible. Continuously expand knowledge and understanding of community resources and services. Document all client encounters and contracts made on behalf of clients. Complete and submit monthly reports. Maintain comprehensive electronic client files; including but not limited to, client notes, release of information, assessments and other medical documents acquired on behalf of the client. Document activities, service plans, and outcomes achieved by client in an effective manner. Facilitate communication and coordinate services between providers and clients. Coordinates and monitors services, including comprehensive tracking of clients' compliance in relation to care plan objectives. Work collaboratively and effectively within a team. Build and maintain positive working relationships with the clients, providers, nurse case managers, agency representatives, supervisors and office staff.

Must be organized, possess strong oral and written communication skills, excellent time management skills, and strong customer service skills. A strong attention to detail, ability to perform data entry with a high level of accuracy, and ability to work effectively with a diverse range of clients with various health and social service needs essential. Proficiency using computers and computer software required. A valid Ohio Driver’s License, valid auto liability insurance, the ability to maintain auto liability insurance, and the ability to maintain insurability under the County’s vehicle insurance policy are necessary. Must be able to carry, move, and push files and/or packages up to 25 pounds unassisted and up to 50 pounds with assistance. A High School Diploma or equivalent; previous experience performing patient assessments, care coordination, and/or data entry into a software system in an office environment; and proficiency in Electronic Medical Records (EMR) systems and Microsoft Office are preferred.
