NOW HIRING: Care Coordination and Addiction Services Supervisor (Partially Grant-funded)

The Care Coordination and Addiction Services Supervisor position will develop, implement and coordinate evidence-based and evidenced informed injury prevention and addiction non-clinical services including policy, system and environmental change strategies, programs, processes and interventions to support community based surveillance and prevention activities. This position will provide oversight in the development, coordination and facilitation of community engagement and collaboration to identify and address disparities and social determinants of health related to poor health outcomes. This position assists/coordinates with writing, editing, and collecting documentation to assure full compliance with grant deliverable outcomes and assists with identifying and approving grant expenditures.

Duties include:

Provides direct and indirect supervision, coaching, and oversight to direct and indirect staff with performance evaluations, mentoring and discipline. Supervises addiction services, Pathways HUB, Tobacco Cessation, Injury Prevention section. Leads candidate reviewing, interviewing and providing hiring recommendations.
Utilizes management tools in the development and monitoring of section activities, including partner outreach, program service planning and delivery, referrals and linkages to community-based organizations, documentation, and confidentiality;
Offers guidance and support in identifying resources that support addressing social determinants of health; Identifying barriers to care and services in areas such as education, transportation, housing understanding procedures and language barriers;
Determines resources required for projects and the most feasible and cost effective methods to gather data; develops work plans; assigns priorities and time limits;
Conducts quality improvement reviews; develops and implements action plans to improve effectiveness and efficiency of staff;
Manages resource planning, budgetary review, and resource allocations. Leads the division workforce training and development initiatives, research review processes, and related initiatives.
Other related duties as assigned.

Requirements:

Master’s degree in public health, mental health and addiction, public administration or related field;
Two to three (2-3) years of direct experience working in public health field;
Experience in grants management, federal grant experience preferred;
At least one (1) year of supervisory experience or equivalent (e.g., leadership);
Must possess a valid driver’s license, and be insurable through FCPH’s provider; some evening and weekend hours required;
Non-smoker.

Hiring Salary: $25.60/hour - $32.00/hour. This is an exempt position.
