NOW HIRING: Care Coordination and Addiction Services Supervisor (Partially Grant-funded)

The Care Coordination and Addiction Services Supervisor position will develop, implement and coordinate evidence-based and evidenced informed injury prevention and addiction non-clinical services including policy, system and environmental change strategies, programs, processes and interventions to support community based surveillance and prevention activities. This position will provide oversight in the development, coordination and facilitation of community engagement and collaboration to identify and address disparities and social determinants of health related to poor health outcomes. This position assists/coordinates with writing, editing, and collecting documentation to assure full compliance with grant deliverable outcomes and assists with identifying and approving grant expenditures.

Hiring Salary: $25.60/hour - $32.00/hour. This is an exempt position.
