AOHC Awards – Please Submit Nominations by August 1

Complete guidelines and nomination forms are here, but here is a brief description of each award:

AOHC PUBLIC HEALTH DISTINGUISHED SERVICE AWARD
Purpose: This award was established to recognize a member of AOHC. The award recognizes a public health official that has:

Eligibility: Member with a minimum of five continuous years of membership.

AOHC PUBLIC HEALTH GUARDIAN AWARD
Purpose: This award was established to recognize any individual for having initiated a new program that improved public health in Ohio. This award recognizes outstanding, significant and innovative activities and programs resulting in a positive impact on public health.

Eligibility: Private citizens, employees of health-related organizations, as well as state and local health district employees, whether members or nonmembers of AOHC, are eligible to receive this award.

AOHC PUBLIC HEALTH LIFE MEMBER AWARD
Purpose: This award was established to recognize an individual who has retired from public health. The award recognizes health commissioners or chief executive officers who have, over the course of their career:

Eligibility: Individuals who are retired from the position of health commissioner or chief executive officer who were members of AOHC at the time of their retirement.
